A. INDIVIDUAL ACCOUNT
  • 3pcs. 2×2 or 1×1 ID Picture
  • Photocopy of One (1) Primary or Two (2) Secondary or Tertiary IDs (Original Copy of ID submitted should be presented for Validation)
  • ₱5,000.00 Initial Deposit
B. PANGARAP SAVINGS ACCOUNT
  • 3pcs. 2×2 or 1×1 ID Picture
  • Photocopy of One (1) Primary or Two (2) Secondary or Tertiary IDs (Original Copy of ID submitted should be presented for Validation)
  • No minimum initial deposit.
  • ₱50,000.00 Maximum Deposit.
C. FOR CORPORATION / ASSOCIATION ACCOUNT
  • Certificate of Registration (CDA/SEC/DOLE/DTI)
  • Articles of Incorporation, Cooperation, and By-Laws
  • Notarized Board Resolutions (Designating CBNV as Depository Bank indicating the Authorized Signatories with Specimen Signatures duly certified by the Secretary & Attested by the Chairman)
  • Permit to Operate such as Barangay/Mayor’s Permit
  • Tax Identification Number of the Entity
  • Tax exemption certificate if applicable
  • General Information Sheet
  • List of Major Stockholders/Shareholders and their Stockholdings (Holders of more than 10% of the total stockholdings/shareholdings of the institution or top 10 holders if 10% and below)
  • Latest List of Board of Directors and Officers (To be updated yearly or when there are changes)
  • Photocopy of Valid ID with Signature and 2×2 ID Picture for the Signatories (Original Copy of ID Submitted should be presented for Validation)
  • ₱10,000.00 Initial Deposit

  • GSIS or SSS-issued Unified Multi-purpose Identification Card (UMID)
  • PhilHealth ID (Digitized PVC)
  • Digitized Postal ID from Philippine Postal Corporation or Philpost
  • Voter’s ID or Voter’s Certificate from Commission on Election (COMELEC)
  • NBI Clearance from the National Bureau of Investigation 
  • Senior Citizen ID from Local Government Unit (LGU)
  • OWWA ID from Overseas Workers Welfare Administration (OWWA)
  • Persons with Disabilities (PWD) ID from Local Government Unit (LGU)
  • IBP ID from Integrated Bar of the Philippines
  • GOCC (Government-Owned or -Controlled Corporation) and Government Office ID from Government agencies (DepED ID, DENR ID, PNP ID, AFP ID, etc.)
  • HDMF ID (Pag-IBIG)
  • School ID (For Students from the current School or University)
  • Alien Certification of Registration or Immigrant Certificate of Registration
  • Philippine National ID (Either digitized or temporary printed copy)

  • Company ID issued by Private Entities or Institutions registered with or supervised, or regulated either by the BSP, SEC, or ICBarangay Certification
